(Erol Otus, from AD&D Deities & Demigods, TSR, 1980. This is from a later printing without the Cthulhu and Melnibone mythoi. The picture is dated ‘81 so perhaps this art was added to the later printings to fill a blank page?) (Edit — Yep, this is a replacement picture from a third printing in 1981.)
